Coverage for individuals members is taken care of by the contractual agreement between the district and the CTA. No further payment is necessary. Coverage for family is either $275 for employee + 1 dependent, o $450 for employee + 2 or more dependents. The best place to start is to check the list of providers located on the Dental Provider List link on the Dental page. Members are also encouraged to share a copy of the Dental Schedule of Benefits with their dentists to see if they would be willing to accept the Schedule benefit payment as payment in full. Dentists that do so can then be listed on the Dental Provider List for the CTA Welfare Fund. Cleaning of teeth (prophylaxis) is covered twice during each plan year. The best way to find out if an optician accepts our plan is to bring a copy of our vision schedule of benefits to your doctor and see if they consider it full payment. Our plan covers one full eye exam per calendar year and one pair of glasses. Depending on your optician, other benefits are available. Certain opticians have created specific agreements with the Welfare Fund and offers more services as part of the plan. . |

Answer -- Every plan year runs from July 1st to June 30 of the following year. Example our current plan year is for expenses incurred between July 1, 2010 to June 30, 2011. Also understand that expenses that occur within that plan year can be sent in for reimbursement up to three months past the end of the plan year. |
